Basic Information

Please fill out the information for each golfer below. Confirmations and other pertinent details will be emailed to the addresses below. The cost is $100 per person, which includes includes 18 holes, cart, lunch and library donation. You may sign up with fewer than four golfers. The 2009 tournament will feature a 9:00 a.m. shotgun start.
